Request for a Quote Disclosure
I hereby request First Financial Insurance
provide me with a quote for insurance, and I authorize the
bank to submit a request for a quote to First Financial Insurance.
I understand that this is not an application for insurance,
but merely a request for a quote to obtain insurance from First
Financial Insurance. I will be required to complete an application
for insurance and meet all the qualifications required. I have
been advised that the bank is not associated with First Financial
Insurance, that the bank does not render any opinion or advice
on the insurance products offered by First Financial Insurance
or another insurance agency has no effect on the terms of a
loan or its approval. I understand the insurance products and
benefits of a loan or its approval.
I understand that insurance products and benefits offered through
First Financial Insurance are not FDIC insured. Insurance contracts
are not obligations of the bank and are not guaranteed by the
bank.
